Welcome to GI North
At GI North, we value our patients’ time and want to streamline the check-in process at both our clinic and the Endoscopy Center.
To do this, we require our patients to update demographic information and medication lists at least once every two years, if not more frequently, depending on prescription updates or recent diagnoses. All our forms are available here and can be downloaded and printed to fill out in the comfort of your home. We ask that you please bring your completed forms with you to your visits to reduce tedious check-in or procedure delays.

Please call our office for any questions or if you have any issues with Form Dr. We encourage patients that have issues filling out the forms to print them out and email them to info@gi-north.com or bring them with them to their appointment.
